Strategic partnerships enable value-based care for independent practices

Dr. Douglas Magenheim, MD, MBA, FACP runs the kind of practice many providers and patients dream about. At My Doctor LLC, a concierge practice in Cincinnati opened by Dr. Magenheim in 2003, he controls appointment lengths, ensures patient engagement feels personal, and delivers care exactly when and how patients need it.

Even in this context, where patient care feels human-centric, the operational pressures are relentless. "It's like drinking out of a firehose," Dr. Magenheim says, describing the constant flow of administrative work that competes for attention with patient care.

In the past several years, Dr. Magenheim faced an operational challenge. He knew his practice was creating value in their clinical outcomes, but the clinical value delivered wasn’t translating into value returned through his revenue, and the administrative load of delivering that care felt increasingly burdensome.

In 2025, My Doctor LLC partnered with Aledade, a physician-led value-based care company, to participate in risk-sharing arrangements that would finally reward their quality outcomes. The Aledade EHR Overlay integration with athenaOne created a seamless workflow that transformed how physicians at My Doctor LLC identified and addressed care gaps.

"The overlay tool helps us understand where those gaps of care are in real time while we're using the chart, as opposed to getting a list of things we need to go back and do when patients aren't there," Dr. Magenheim explains. This real-time visibility allowed providers to identify and address care gaps during the visit itself, instead of in retrospective quality reporting.

Seamless Behind-the-Scenes Integration

The technical integration of Aledade into athenaOne required little to no effort from My Doctor LLC's team. Dr. Magenheim describes the athenahealth side of the partnership as "completely invisible" to daily operations because the systems communicate smoothly in the background without any workflow disruptions, system conflicts, or training requirements.

According to Magenheim, the integration might as well have been a flip of a switch: "We contacted [athenahealth] once to tell them we were partnering with Aledade. Other than that, we've had educational meetings and no problems with the integration."

This ease of experience extends to the clinical team, as well. The practice has already implemented new programs like transitional care management, which they'd never billed for previously, creating immediate cost savings.

Financial sustainability meets clinical excellence

This partnership has empowered Dr. Magenheim to address both sides of the independent practice equation, as he can both deliver better care and get paid fairly and accurately for it. Dr. Magenheim sees this as essential for his practice's sustainability and growth.

"Financially, it should be rewarding because we're doing the things we need to do. The shared revenue will help us be more profitable and probably recruit more physicians to a process we think works really well," he says.

This approach builds a model that can attract both new physicians and new patients while maintaining independence, financial success, and high-quality patient care.

Looking Forward: Quality and Revenue Alignment

As My Doctor LLC moves into their first full year with the partnership, Dr. Magenheim is excited about the opportunity ahead. At his organization, he aims to demonstrate that it’s possible to deliver excellent care and get paid for the outcomes while remaining independent.

"We're looking forward to closing those quality gaps and care gaps and being reimbursed for doing good care,” Dr. Magenheim says. “That's what this three-way partnership will allow us to do.”
